The Community Alternatives Program
for Disabled Adults (CAP/DA) is a Medicaid waivered program that provides home
& community based services to Medicaid recipients 18 years & older who
would otherwise require care in a Nursing Facility.CAP/DA offers an alternative to
institutional placement.
We provide quality care in the
home for those needing assistance with:

Aide Services: Assists with personal care (bathing & dressing), light
housekeeping tasks, & meal preparation

Nursing Services
·Physical,
Occupational, and/or Speech Therapy
·Respite
Care
·Adult

Aids

Medications will be covered my Medicaid
CAP/DA Services are available to
individuals at least 18 years of age who are certified ICF or SNF Nursing Home
qualified by a physician & meet Medicaid eligibility guidelines.
The Community Alternatives Program
for Children (CAP/C) is a special Medicaid community care program that serves a limited
number of medically high risk children (under 18 years of age) who would face
institutionalization in a nursing facility or hospital by providing:

Physical & Occupational Therapy

CAP/C Services are available to
